CMS CY 2027 PFS proposed rule — not final. Comments are due September 14, 2026.

Executive Summary

39Tennessee ZIP redesignations: 20 urban→rural and 19 rural→urban.
35counties flagged: 18 urban→rural only and 17 rural→urban only; 0 mixed.
60counties have no affected ZIP in either CMS Tennessee change file.

How to Read the County Chart

Classification follows the point-of-pickup ZIP on the ambulance claim

A county is flagged when any affected Census ZCTA intersects it. ZIPs may cross county lines, so the same ZIP can appear for multiple counties. “No change” means no ZIP in either official CMS Tennessee change file was linked to that county; it does not mean every ZIP in the county has the same urban/rural status.
